Import district list (CSV)

Needs a header row with a district-name column (e.g. name or District Name). Also recognized: AD name, address, website, phone, email, assistant name — under most natural spellings. Rows that share a district name are merged into one district, with any extra contacts kept as a note rather than creating duplicate districts.
